Admission Requirements

All residents are required to provide the following documentation upon admission:

  1. Social Security card
  2. Medicare card (if applicable)
  3. Additional insurance information or cards
  4. Copy of responsible party’s Power of Attorney (if applicable)
  5. Copy of Living Will or Health Care Power of Attorney (if applicable)
  6. Names and addresses of people to contact in an emergency

Admission from Hospital:

  1. In North Carolina, FL2 (with approval numbers) and PASARR
  2. In Kentucky, medical information and PASARR
  3. In Virginia, prescreening UAI and PASARR
  4. MAR – Medical Administration Record*
  5. IV Administration Record (past 14 days)*
  6. Completed history and physical form*
  7. Chest x-ray or PPD
  8. Lab reports if applicable
  9. Therapy notes if applicable
  10. Signed discharge summary or transfer form with orders signed by the physician

* should be provided prior to admission

Admission from Home, Rest Home, Assisted Living or Personal Care Facility:

  1. In North Carolina, FL2 (with approval numbers) and PASARR
  2. In Kentucky, medical information and PASARR
  3. In Virginia, prescreening UAI and PASARR
  4. Completed history and physical form (within 5 days of admission)
  5. Signed physicians orders
  6. Chest x-ray or PPD (within 3 months)
  7. Home health notes (if applicable)

If admission is within two weeks of a hospital discharge and under Medicare Part A, requirements will follow under “Admission from Hospital.”

Admission from another Nursing Facility:

  1. In North Carolina, FL2 (with approval numbers) and PASARR
  2. In Kentucky, medical information and PASARR
  3. In Virginia, prescreening UAI and PASARR
  4. Completed history and physical form (current or updated by physician)
  5. Lab reports if applicable
  6. Transfer form with orders signed by physician

If transferring under Medicare Part A, requirements will follow under “Admission from Hospital.”
